SD Primary Care Provider (PCP Program)

The PCP Program is designed to improve access, availability, and continuity of care while reducing inappropriate utilization, over-utilization, and duplication of South Dakota Medicaid covered services. Participating recipients select a PCP to be their health care case manager. This program creates a partnership between the PCP and the recipient. The PCP is responsible for providing or directing most services provided to the recipient. Mandatory for Aged, Blind or Disabled Children or Adults.
